RPC will also be integrated into the emergent care system of Henderson. We will act to alleviate demand for ED services by shifting lower acute patients to a less resource-intensive environment. Emergent patients entering our clinic will be triaged and stabilized.

Meanwhile, ambulance service will be located at Rose Petal Clinic to transfer emergent patients to the St. Rose Dominican ED. Also, locating ambulances at RPC provides superior response times to the far ends of the city. Our facility design must meet the needs of clinicians and consumers.

We understand that consumers invariably associate the quality of healthcare services with the aesthetics of the site of care. At RPC, our facility will be designed to blend into the local architecture to be a part of our community. Design will focus more on clinical efficiency and safety.

Our urgent care centre will be designed to accommodate urgent patients who must be continually monitored by our staff. Also, the waiting area will remain in the line of sight to a receptionist to quickly respond to patient needs. Our exam room will be located around the central nursing/physician station to allow for continual monitoring.

Additionally, the central workstation will provide our staff the opportunity to remain in constant contact, promoting communication and teamwork. A single entry point will be available at the front of the facility with a separate exit point. An ambulance entrance will also be provided for quick exit by the EMTs located onsite.

At the other end of the acuity spectrum, most emergent patients would be better served in an inpatient unit, but many are forced to board in the ED because beds are unavailable. Triaging patients to appropriate site of care properly allocates resources to meet patient acuity and result in better clinical outcomes.

How to Start an Urgent Care: Tips for Clinic Design and Setup

Some of the most difficult decisions in figuring out how to start an urgent care revolve around selecting, designing and setting up the physical space.

Acquiring or constructing space for your new practice can be expensive, so you should know what you will need and can afford to meet requirements, while planning for future additions if possible.

Clinic Space

General guidelines for sizing indicate that for each provider in your practice you will need four to five exam rooms (9′ x 9′), one radiology suite, a centralized nursing area, and one procedure room. Your clinic must have a welcoming reception area, medical record/business office designed for privacy, an enclosed break room, doctor’s office and adequate storage space. In addition, you will need restrooms in your waiting room, for your staff, for clinical use and for drug screening (preferably one designed with Department of Transportation rules in mind).

It’s best to have shared space for reception, lab, and nursing areas when blending urgent care, occupational health, physical therapy, and/or primary care; however, planning for the future development of any of these services may require careful planning early on in order to reduce the construction cost and prevent possible poor patient flow.

Areas for your patients should be comfortable and efficient, while allowing for patient privacy. Your waiting room should be inviting with media, Wi-Fi, and a centralized nursing station. and discharge routing past the business office or reception desk. For patient privacy and efficiency, it’s helpful to have the x-ray suite near the procedure room.

Equipment Your practice and budget will determine what your radiology requirements will be. A CR Suite (Computerized Radiology) can range between $65,000 for a with a picture archiving and communication system (PACS). A DR Suite (Digital Radiology) can cost up to $95,000, also with PACS. You must order a PACS to manage x-ray images in your new urgent care. Also remember, a physicist must work with you to finish plans and installation of your radiology suite, avoiding issues during final inspection.

Other capital equipment you will need to consider includes exam tables, diagnostic devices, and procedure lights. These can be leased, which provides greater tax deductions and avoids operational capital you might need for something else. Or you can purchase these items, with refurbished equipment a viable consideration. You will also want to limit duplicate equipment within the clinic.

Other Considerations You will want to include computer wiring, wireless access, counter space, water, and cabinetry in exam rooms. Also, be sure to include plenty of electrical outlets. If you are building your space, verify contractors’ references and don’t hire without looking at their completed work. Once building starts, regularly visit the site of your new urgent care and keep contractors on schedule and on design. However, recognize that there could be delays.

Start your urgent care center planning early with a well-designed business plan including a comprehensive timeline and a great location visible to passing traffic. Move on to financing, designing space and purchasing equipment. Be sure to start your credentialing and contracting, marketing and EMR/PM software selection 8 to 12 months before your projected opening date. You can be successful.

Emergency medical care is provided free to all foreign nationals in case of life-threatening conditions that require immediate medical treatment. You will be given first aid and emergency surgery when necessary in all public health care facilities. Any further treatment will be free only to people with a Compulsory Medical Insurance, or you will need to pay for medical services. Public health care is provided in federal and local care facilities. These include 1. Urban polyclinics with specialists in different areas that offer general medical care. 2. Ambulatory and hospitals that provide a full range of services, including emergency care. 3. Emergency stations opened 24 hours a day, can be visited in a case of a non-life-threatening injury. It is often hard to find English-speaking staff in state facilities, except the largest city hospitals, so you will need a Russian-speaking interpreter to accompany your visit to a free doctor or hospital. If medical assistance is required, the insurance company should be contacted before visiting a medical facility for treatment, except emergency cases. Make sure that you have enough money to pay any necessary fees that may be charged.

Travelers need to arrange private travel insurance before the journey. You would need the insurance when applying for the Russian visa. If you arrange the insurance outside Russia, it is important to make sure the insurer is licensed in Russia. Only licensed companies may be accepted under Russian law. Holders of a temporary residence permit or permanent residence permit (valid for three and five years respectively) should apply for «Compulsory Medical Policy». It covers state healthcare only. An employer usually deals with this. The issued health card is shown whenever medical attention is required. Compulsory Medical Policyholders can get basic health care, such as emergencies, consultations with doctors, necessary scans and tests free. For more complex healthcare every person (both Russian and foreign nationals) must pay extra, or take out additional medical insurance. Clearly, you will have to be prepared to wait in a queue to see a specialist in a public health care facility (Compulsory Medical Policyholders can set an appointment using EMIAS site or ATM). In case you are a UK citizen, free, limited medical treatment in state hospitals will be provided as a part of a reciprocal agreement between Russia and UK.

Ambulances come with a doctor and, depending on the case, immediate first aid treatment may be provided. If necessary, the patient is taken to the nearest emergency room or hospital, or to a private hospital if the holder’s insurance policy requires it.
